Caregiving services, assistance with activities of daily living, nursing services, assistive technology, and even modifications to your house are aspects of in-home care. If you are needing assistance to live safely in your own home and meet income qualifications, you could be eligible to receive in-home care.
What is in-home care?
The primary goal is to assist clients to develop a plan of care that lets them live at home with as much health and safety as possible. NWRC works with those who choose to live in their own homes, as opposed to moving into a nursing home, assisted living, or an adult family home. Our case managers will coordinate and offer assistance to get the needed services, which includes caregiving, nursing, meals, medical equipment, and more.
How do I find caregivers to help me in my home?
The main element of successful in-home long-term care is to find caregivers to provide you with support with daily activities, like bathing, dressing, meal preparation and mobility. Often this is an individual provider who is contracted to serve a particular client. Sometimes it is a worker from a caregiving agency. There are a number of ways to find caregivers in the community and we make sure clients understand all possible options.
How will I know what kinds of other services I may be eligible for?
Case managers help put together a care plan with your input. Case managers work with you to thoroughly assess your needs and that helps them to determine how much help is necessary. They also determine what other services you may be eligible for, such as nursing services or Meals on Wheels. Case managers work with each client individually to discuss what is needed for their own unique care plan and how best to find the resources.
Funding
NWRC maintains contracts with providers who offer of one of the broadest arrays of services and supports in the state. NWRC makes sure those providers meet all the state and federal laws, regulations, and other performance standards that apply to the services they provide.
